Agile delivery failures are usually explained with comfortable excuses. The backlog was unclear. The scope changed. The estimates were wrong. The architecture was fragile. The process wasn’t followed closely enough.
In real delivery environments, especially complex or hybrid ones, those explanations rarely hold up for long.
